Calcitonin

Calcitonin is a 32-amino acid peptide hormone produced by parafollicular C-cells of the thyroid gland that inhibits osteoclast-mediated bone resorption. Salmon calcitonin (sCT), which has 40-50 times greater potency than human calcitonin, is the therapeutic form approved for osteoporosis and Paget's disease. Available as nasal spray (Miacalcin/Fortical) and injection, calcitonin directly reduces bone breakdown, provides analgesic effects for bone pain, and helps maintain calcium homeostasis. While largely superseded by bisphosphonates and denosumab for osteoporosis, calcitonin retains a role for acute vertebral fracture pain management.

Degarelix

Degarelix (Firmagon) is a GnRH (gonadotropin-releasing hormone) receptor antagonist — a synthetic decapeptide that directly blocks the GnRH receptor on anterior pituitary gonadotroph cells. FDA-approved for advanced prostate cancer, it produces immediate and profound suppression of testosterone to castrate levels without the initial testosterone flare seen with GnRH agonists (leuprolide, goserelin). Unlike GnRH agonists which initially stimulate LH/FSH release before causing receptor desensitization, degarelix competitively blocks GnRH-R from the first dose, achieving castrate testosterone levels within 3 days. This is clinically important in advanced prostate cancer where testosterone flare can cause tumor flare, spinal cord compression, or urinary obstruction.

Melanotan I

Melanotan I (afamelanotide) is a linear 13-amino acid analog of α-MSH developed at the University of Arizona with enhanced MC1R selectivity and metabolic stability compared to the native hormone. The key modification is substitution of methionine-4 with norleucine (Nle) and phenylalanine-7 with D-phenylalanine (D-Phe), providing resistance to enzymatic degradation. Approved by the EMA as Scenesse (2014) for prevention of phototoxicity in adults with erythropoietic protoporphyria (EPP), afamelanotide is the only approved melanocortin-based tanning agent. It stimulates eumelanin production without UV exposure, providing photoprotection for patients with extreme UV sensitivity. Unlike Melanotan II, afamelanotide is relatively selective for MC1R with minimal sexual or appetite effects.

Pentosan Polysulfate

Pentosan polysulfate sodium (PPS) is a semi-synthetic sulfated polysaccharide derived from beech wood hemicellulose, structurally similar to heparin and glycosaminoglycans. While not a peptide, it is included in peptide databases due to its use alongside peptides in regenerative medicine. FDA-approved as Elmiron (1996) for interstitial cystitis/bladder pain syndrome, PPS has glycosaminoglycan-like properties that restore the bladder's protective mucous lining (GAG layer). PPS has also been investigated for osteoarthritis (approved in Australia as Pentosan Equine for veterinary use), blood clotting disorders, and prion diseases. Recent concern about maculopathy (retinal damage) with long-term use has impacted its clinical use for interstitial cystitis.
